Application for Re-take or Make-up Course of 2019 fall  Semester

※ Switching Sections (Graduating students and transfer students only)

1. Fill out the attached application form.

2. Please be sure to provide the full academic transcript (You can go to the 2^nd floor of the Administration Building to print it by yourself). The transcript will be returned after the assistant confirms the grades and level are correct.

3. Please provide the class schedule for the next semester (There’s no need to provide it if you are sure that there’s no class conflict).

4. The application date is from June 24, 2019 to September 6, 2019 (For the application deadline, please refer to the announcement of school calendar).

※ Re-take or Make-up Course

1. Fill out the attached application form.

2. Please be sure to provide the full academic transcript (You can go to the 2^nd floor of the Administration Building to print it by yourself). The transcript will be returned after the assistant confirms the grades and level are correct.

3. It is acceptable for students to ask their classmates to apply on their behalf.

4. The application date is from June 24, 2019 to September 6, 2019 (For the application deadline, please refer to the announcement of school calendar).

※ Notes

1. Students applying for switching sections or re-take/make-up course must attach the above information, otherwise they will not be accepted.

2. Please apply directly to the assistant of CELT.

3. The official class schedule will be announced before the school starts. Therefore, the deadline is September 6, 2019 (For the application deadline, please refer to the announcement of school calendar).

* It is not acceptable to assign the instructor.

* The general English course cannot be added into your class schedule until the elective course is dropped.

* The general English course cannot be added into your class schedule until the required course is dropped.
